Help! I want to add an artist/song, but I don't know how

This song was actually a hidden track at the end of Feed and Grain, attached to "My Concern."

it comes on after about 4 minutes of silence, and has a really cool feel to it. Here are the lyrics I transcribed a few years ago- hopefully I'll be able to figure out what chords he's playing. The chords should be really easy to figure out for someone with talent.
